Older Adult Care Management
Our niche is having clinicians who have a unique combination of education in psychology and social work care management, along with years of experience providing care management and counseling. We provide Care Management in the comfort of our Older Adults’ private homes, retirement communities, assisted living facilities and nursing homes. We support Older Adults and their responsible parties by connecting them to resources, overseeing those resources, and continuously assessing the needs of the individual(s) to assist them in achieving their goals as they live out their life.
Counseling
Our Therapists are Licensed Clinical Social Workers and Care Managers. They have experience counseling adults of all ages, families, and caregivers. Their training has led them to support people who are experiencing a life transition which is causing them to feel overwhelmed and stuck. Our Therapists support clients in moving forward and overcoming the challenges they are experiencing. This is done by better understanding the situation that is causing them to feel the way they are, to create small goals to help them achieve larger goals, and therefore have a more peaceful future.
